Huron County Public Health (HCPH) is able to assist the public in obtaining certified birth and death certificate copies via the following convenient options:

HCPH has statewide access to birth certificates and county-wide access to death certificates.
Due to a change in Ohio law, only certified copies of birth/death certificates can be issued. We are not authorized to issue or fax uncertified copies of birth certificates.

Huron County Public Health (HCPH) now has the option for clients to order certified copies of birth and death certificates online. Simply scan the QR code below for the certificate you are seeking, or click on the link beneath the QR code, and follow the instructions listed on each form.

Clients are currently limited to ordering 10* certified copies per transaction.
*If ordering more than 10 certificates, please call the Office of Birth and Death Records at 419-668-1652 ext. 244.

Our walk-in customer counter is located at 28 Executive Drive, Norwalk, OH 44857 to order same-day certified birth and death certificate copies quickly and easily.

Please remember there is an additional $5.55 charge to mail your certificate back. Please send payment and completed application to our office at

Fees

The base fee for all certified certificates that are picked up or mailed is $25.00 per copy. Checks and money orders should be made payable to Huron County Public Health.

All certified birth and death certificates that are mailed are only mailed utilizing certified mail. The fee for certified mailing is $5.55 plus the cost of each certified certificate.

Cash, money orders, and credit/debit cards are also accepted. An additional convenience fee applies to credit and debit card orders.
