Vitals - Vital Statistics
The Huron County General Health District has birth and death certificates from December 1908 to the present for those individuals that were born or who died in Huron County or Bellevue.
Records from 1867 through 1908 are stored at the Huron County Probate Court, phone (419) 668-4383.
Birth/Death Certificate Requests
Same Day Service
For same day service, come to the office, located in the lower level of the county administration building at 180 Milan Avenue (State Route 250). The cost of a certified copy is $25.00 (cash, check or money order). Debit cards or Credit Cards are accepted but there is an additional fee of $3.00.
By Phone/Mail In
Download the Application for Certified Birth/Death Certificate below to request a record by mail and if payment is by check or money order. Due to a change in Ohio law, only certified copies of birth/death certificates can be issued. Debit cards or Credit Cards are accepted but there is an additional fee of $3.00.
For records outside of our district, but within Ohio, the certificate can be obtained from the Ohio Department of Health, Vital Statistics, P. O.
